Registrar Forms
Below are direct links to the forms available through the Office of the Registrar. Some links will redirect you to an online ticketing system, and others are fillable PDFs. Please note that although some forms are fillable, an electronic signature is not valid unless the document is sent directly from your WesternU email address. Please e-mail the completed form to registrar@westernu.edu.
- Enrollment Verification – Verifies current (and past, if requested) enrollment dates, program, and expected graduation date.
- Degree Verification – Verifies enrollment dates, program, and graduation date for alumni.
- Jury Summons Letter – Request to have jury duty postponed (the Registrar’s Office does not guarantee excusal from jury duty).

- Directory Information Waiver Form – The Family Educational Rights and Privacy Act is a federal law addressing the privacy of students’ educational records. WesternU may disclose directory information. Students have the option to opt out. (If you are unable to access the MyPortal, please download, complete, and email the following form to registrar@westernu.edu – Directory Information Waiver Form)
- Student Data Release Form – Students may grant permission to WesternU to release information related to financial issues and academic information to specific individuals (parents, partners, spouses, etc.). If you are unable to access the MyPortal, please download, complete, and email the following form to registrar@westernu.edu – Student Data Release Form.
- Authorization to Release Item – Students may grant permission to WesternU to release academic records (diploma, official transcripts, verification of enrollment) to specific individuals. If you are unable to access the MyPortal, please download, complete, and email the following form to registrar@westernu.edu – Authorization to Release Item.
- Student Records Request Form – Previous WesternU Students Only. Submit this form to request access to specific documents or your entire student file. This form is not intended for students requesting their unofficial transcript or an enrollment/degree verification. Current students must locate and submit this form directly from the My Portal (within the Student Authorization Forms card).
- Add/Drop Form – Students dropping all classes for a term, must complete a withdrawal form and meet with their College.
