Some links will redirect you to an online ticketing system, and others are fillable PDFs. Please note that although some forms are fillable, an\u00a0electronic signature is not valid\u00a0unless the document is sent directly from your WesternU email address. Once you have filled out the form, please print and sign. You can also e-mail the form with your signature to\u00a0<a href=\"mailto:registrar@westernu.edu\">registrar@westernu.edu.<\/a><\/p>\n<ul>\n<li><a href=\"https:\/\/support.westernu.edu\/TDClient\/1848\/Portal\/Requests\/ServiceDet?ID=39137\" target=\"_blank\" rel=\"noopener\">Enrollment Verification<\/a> &#8211; Verifies current (and past, if requested) enrollment dates, program, and expected graduation date.<\/li>\n<li><a href=\"https:\/\/support.westernu.edu\/TDClient\/1848\/Portal\/Requests\/ServiceDet?ID=39140\" target=\"_blank\" rel=\"noopener\">Degree Verification<\/a> &#8211; Verifies enrollment dates, program, and graduation date for alumni.<\/li>\n<li><a href=\"https:\/\/support.westernu.edu\/TDClient\/1848\/Portal\/Requests\/ServiceDet?ID=39113\" target=\"_blank\" rel=\"noopener\">Jury Summons Letter<\/a> &#8211; Request to have jury duty postponed (the Office of the Registrar does <em>not<\/em> guarantee excusal from jury duty).<\/li>\n<li><a href=\"https:\/\/support.westernu.edu\/TDClient\/1848\/Portal\/Requests\/ServiceDet?ID=39032\" target=\"_blank\" rel=\"noopener\">Name Change Request<\/a> &#8211; IT strongly advises you from changing your email to match your new name. E-mail change can create issues with permissions to various online resources.<\/li>\n<li><a href=\"https:\/\/stagewp.westernu.edu\/media\/registrar\/pdfs\/directory-waiver.pdf\" target=\"_blank\" rel=\"noopener\">Directory Information Waiver Form<\/a> &#8211; The Family Educational Rights and Privacy Act is a federal law addressing the privacy of students\u2019 educational records. WesternU may disclose directory information. Students have the option to opt out.<\/li>\n<li><a href=\"https:\/\/stagewp.westernu.edu\/media\/registrar\/pdfs\/student-data-release.pdf\" target=\"_blank\" rel=\"noopener\">Student Data Release Form<\/a> &#8211; Students may grant permission to WesternU to release <em>information<\/em> related to financial issues and academic information to specific individuals (parents, partners, spouses, etc.).<\/li>\n<li><a href=\"https:\/\/stagewp.westernu.edu\/media\/registrar\/pdfs\/authorization_to_release_item.pdf\" target=\"_blank\" rel=\"noopener\">Authorization to Release Item<\/a> &#8211; Students may grant permission to WesternU to release academic <em>records<\/em> (diploma, official transcripts, verification of enrollment) to specific individuals.<\/li>\n<li><a href=\"\/media\/registrar\/student-records-request-form.pdf\">Student Records Request Form<\/a> &#8211; <strong>Previous WesternU Students Only. <\/strong>Submit this form to request access to specific documents or your entire student file.
