All students must send documentation showing they have received this vaccine to <a class=\"cta-arrow cta-arrow--red\" href=\"mailto:stu-emphealth@westernu.edu\">stu-emphealth@westernu.edu<\/a> to avoid a hold on their account. If your college has an earlier due date, then you must follow that due date. The only form you will need to renew every year is the <a href=\"https:\/\/stagewp.westernu.edu\/media\/health\/pdfs\/tb-symptoms-checklist-form.pdf\">TB Symptoms Checklist Form.<\/a><\/li>\n<li><strong>TB skin test (PPD):<\/strong> submit 1 TB skin test before it expires. If you let it expire, two (2) separate TB skin tests, at least 7 days apart from each other, are required.<\/li>\n<li><strong>Chest x-ray<\/strong> (only if you have history of positive skin test\/positive Quantiferon): this test is valid for 4 years in the university.
